Comment field in Prize Redemption Form

Some of the prizes that we offer our team are very generalized, like a charitable donation. I think it would be beneficial to add a comment box to the prize redemption form for the user to add comments. For example, the ability to designate the charity for the donation, or to request splitting the value of a $20 gift card into 2 $10 cards.
