Need multiple questions in the NPS Campaigns

We need more than one question to be sent out on the email for the NPS campaigns to better understand and gage our clients. 

Question 1
Please rate the quality of the IT services and solutions provided to your company. (0=terrible, 10=excellent)

Question 2

How satisfied are you with the level of service provided by our IT team? (0=not at all, 10=very satisfied) Question 3 How likely is it that you would recommend TeamLogic IT to a friend or colleague? (0=not at all, 10=very likely)

  I would go farther and ask that a multiple ranking NPS be created that is completely customizable.
